WFH price list and order form

This is a publication order form for members of the WFH and for non-members. Members receive discounts on WFH publications.

Please note that all orders must be prepaid. Prices include shipping and handling. If paying by money order or by cheque (in U.S. dollars), print a copy of this form and mail to: 1425 René Lévesque Boulevard West, Suite 1010, Montréal, Québec H3G 1T7 Canada.

The WFH may be able to subsidize the cost of some publications depending on individual circumstances. Please contact WFH headquarters for more information.

NOTE: National Member Organizations (NMOs) and Hemophilia Treatment Centres (HTCs) are entitled to one free copy of WFH publications (excluding Living with Haemophilia). Shipping and handling paid by the NMO/HTC; please specify the name of the NMO/Treatment Centre on the form.
